METHODS
Design:
randomised controlled trial (Womens Health Initiative Dietary Modification Trial).

Setting:
40 clinical centres in the US.
Participants:
48 835 women who were 5079 years of age (mean age 62 y, 81% white), were postmenopausal, and consumed a diet with fat intake of
32% of daily total calories. Exclusion criteria: previous breast cancer, cancers other than non-melanoma skin cancer in past 10 years, illness with predicted survival <3 years, and impediments to adherence (eg, alcoholism).
Intervention:
low fat dietary intervention (n = 19 541) or usual diet (n = 29 294). Women in the low fat group received educational sessions in which they were encouraged to reduce total dietary fat to 20% and to eat
