Design:

cohort study (Hyperglycemia and Adverse Pregnancy Outcome [HAPO] study) with follow-up to delivery.

Setting:

15 centres in the US, Canada, Barbados, UK, the Netherlands, Israel, Thailand, Hong Kong, Singapore, and Australia.
